Starting Solids and Baby-led Weaning: A Pediatrician's Guide to First Foods

The first time you offer your baby a spoonful of something that is not milk, you are joining a ritual as old as parenting itself and a surprisingly modern debate. A generation ago, rice cereal at four months was standard advice. Today the guidance has shifted, the science has caught up, and parents face a fork in the road that did not exist for their own mothers: purees or finger foods, four months or six, peanut butter now or peanut butter later. The reassuring part is that starting solids is far less about getting every choice perfect and much more about reading the small, specific signals your baby is already sending.

When Your Baby is Ready for Solids

The best timing for first foods is determined by your child’s development, not a date on a calendar. The American Academy of Pediatrics recommends waiting until about six months, keeping breast milk or formula alongside first foods, and watching for readiness signs rather than a birthday: your baby can sit upright with little support, holds their head steady, opens their mouth when food comes near, and has outgrown the tongue-thrust reflex, the early tendency to push food back out with the tongue.

These cues are part of your baby's broader developmental milestones, which is why sitting and reaching for objects often show up right when feeding readiness does. Every baby is a little different, and babies born prematurely may need extra weeks. If your baby cries or turns away, treat that as a no for now, go back to milk feeds, and try again in a week or two.

Your Baby's First Foods

There is no single correct first food. Pediatric guidance no longer prescribes a rigid order, so an iron-rich puree, mashed avocado, or a soft strip of ripe banana are all good places to start. What matters more than sequence is nutrition. Around six months, the iron stores your baby built up before birth start to run low, so iron-rich foods such as pureed meats, lentils, beans, and iron-fortified cereals deserve a starring role early on.

Start small, with a half spoonful or a single soft piece, and expect mess, funny faces, and outright rejection at first. A new eater needs only a tablespoon or two of any food, building up gradually as interest grows. Textures can advance on a rough timeline: thin purees and soft mash early, thicker mash and soft finger foods by eight to ten months, then small chopped pieces of family food near the first birthday. Breast milk or formula stay the main source of nutrition for the first year, so think of first foods as practice and exposure rather than a replacement for milk. There is no need for special equipment or a shelf of jars either; a small spoon, a few soft whole foods, and some patience cover the basics.

Baby-led Weaning vs. Purees

Once your baby is eating, the biggest decision most parents face is method. The options are spoon-fed purees, baby-led weaning where your baby self-feeds soft finger foods from the start, or a blend of both. Purees give you control over pace and make it easy to get iron-rich foods in early. Baby-led weaning lets your baby set the tempo, practice hand-to-mouth coordination, and join family meals sooner, though it tends to be messier and can be slower to deliver enough iron.

The most common worry is choking. But there’s reassuring news: A trial published in Pediatrics found that babies following a structured, baby-led approach did not choke more often than spoon-fed babies, as long as caregivers avoided hazard foods and supervised meals. Hazard foods are the ones most likely to lodge in a baby's small airway, and they tend to share a few traits: hard, round or coin-shaped, sticky, or firm enough to break off into a slick chunk. Common examples include whole nuts, popcorn, whole or halved grapes, hard raw vegetables like carrot sticks, chunks of raw apple, thick globs of nut butter. The reassuring part is that most of these become safe with a little prep: cut round foods such as grapes into thin lengthwise strips, grate or cook hard produce until it is fork-soft, thin nut butter with water or breast milk, and keep pieces no larger than your baby can manage. Serving meals seated, calm, and always within arm's reach handles the rest.It also helps to know that gagging is not choking. Gagging is a loud but protective reflex that moves food forward in the mouth, and it usually settles as your baby gets more practice. There is no wrong answer between the two methods, and many families comfortably combine them.

Introducing Allergens Safely

For years the advice was to delay peanuts, eggs, and other common allergens–but that’s no longer recommended. Now, experts say to introduce allergens safely through early, regular exposure once your baby has started solids, usually beginning around six months.

This shift is because of a trial that showed that introducing peanuts early to high-risk infants reduced their chance of developing a peanut allergy by roughly 80 percent compared with avoidance.

To introduce peanuts, offer smooth peanut butter thinned with water or breast milk, or you can add peanut powder into a puree. Never offer whole nuts or thick globs, which are choking hazards. For eggs, make sure they're well-cooked. Make sure you introduce only one new allergen at a time so you can watch for any reaction, and keep offering these foods regularly rather than just once. If your baby has severe eczema or a known food allergy, talk with your pediatrician before introducing peanuts, as testing may be recommended first.

Choking Hazards and Feeding Safety

A few foods stay off the menu in the first year. Skip honey until twelve months because of the small risk of infant botulism, a rare illness from bacteria a baby's gut cannot yet handle. Hold off on cow's milk until age one, though yogurt and cheese are fine. Avoid juice as well. You can add some water as you add solid foods in addition to breast milk and formula. Remember to steer clear of choking-shaped foods such as whole nuts, popcorn, whole grapes, hard raw vegetables, and rounds of hot dog. Cut round foods lengthwise, cook hard produce until it is fork-soft, offer pieces no larger than your baby can manage, and always stay within arm's reach during meals.

New foods can also shake up digestion, and a stretch of firmer stools is common when solids begin. Offering water with meals plus fiber-rich fruits and vegetables usually helps, though persistent constipation in children is worth a call. Some refusal is normal too. It can take ten or more tries before a baby accepts a new flavor, so keep offering without pressure. If feeding stays a daily battle or your child gags on nearly everything as they grow, those feeding milestones are worth raising with your pediatrician. Call right away for any sign of an allergic reaction, including hives, swelling, vomiting, or trouble breathing.

A Calm Start Beats a Perfect One

Starting solids is a season of trial and error. Follow your baby's readiness cues, lead with iron-rich foods, introduce allergens early and often, and keep every meal safe and low pressure. The method matters far less than building a calm, curious, lifelong relationship with food.
